A statement on the Everton official website read: "Everton can confirm that it has agreed an eight-figure sum with Fulham for the services of Andrew Johnson.
"This is subject to the player agreeing personal terms and negotiating a medical."
Sunderland and Wigan had also been credited with an interest in the 27-year-old, but Johnson's desire to return to London for family reasons meant Craven Cottage boss Roy Hodgson was always in pole position to sign him.
The forward joined Everton from Crystal Palace for a then club record £8.6m in May 2006 and scored 22 goals in 74 appearances.
